A brighter smile does not always require extensive cosmetic dentistry.

For some patients, professional teeth whitening may be an appropriate conservative option. For others, a white spot on an otherwise healthy-looking tooth may be the issue they actually want to address.

These are different problems and should be assessed differently.

At PreBeau Oral Care in Akasaka, Tokyo, my approach is to preserve natural tooth structure whenever clinically appropriate.

Teeth whitening is different from treating white spots

This distinction is important.

Teeth whitening changes the overall color of natural teeth through professional bleaching.

White spot treatment addresses localized areas of altered enamel appearance.

Whitening does not necessarily remove white spots.

In fact, after whitening, some patients notice existing white spots more clearly because the surrounding enamel becomes lighter.

That is one reason why a proper examination is important before cosmetic treatment.

What causes white spots?

White spots can have different causes.

They may be associated with enamel demineralization, previous orthodontic treatment, developmental enamel changes, or other conditions.

A dentist needs to determine what type of lesion is present before deciding whether treatment is appropriate.

At PreBeau Oral Care, I have a particular clinical focus on ICON resin infiltration for appropriate white spot lesions.

I am also a DMG Icon Certified Instructor and Key Opinion Leader.

What is ICON resin infiltration?

ICON is a resin infiltration technique designed for certain non-cavitated enamel lesions.

One of its major attractions for appropriately selected cases is that it can improve the appearance of certain white spot lesions without conventional drilling.

However, not every white spot is suitable for ICON.

The depth, cause, surface characteristics and clinical appearance of the lesion must be considered.

“No-drill” should never mean “no diagnosis.”

Diagnosis comes first.

What about teeth whitening?

Professional whitening can be an appropriate conservative cosmetic option for patients who want a brighter natural smile.

The appropriate method depends on the patient’s teeth, sensitivity, existing restorations and cosmetic goals.

It is also important to understand that whitening changes natural tooth color but does not whiten existing crowns, veneers or fillings in the same way.
